The shift from "learning as event" to "learning as infrastructure" is something I've watched organizations struggle with in practice. The structural tension you've named is real, completion rates go up, incident rates don't move. The measurement piece is where it tends to collapse. Most orgs are still measuring activity because performance data is harder to build and harder to defend internally. The governance layer you're describing at the end is where I think the real fight happens.
